If you are a new patient, please download and complete the new patient forms before coming to your first visit. This will save time and avoid delays!
*Effective August 1, 2012 the fee for a missed or late appointment will be $50.00. If you have multiple children scheduled, each missed appointment will be charged a $50.00 missed appointment fee. This fee will not be billed to your Insurance Company.
